	<title>Does the Timing of Surgery Predict Clinical and Radiographic Outcomes After Talar Fracture?</title>

	<abstract>Background: Talar fractures are uncommon, usually result from high-energy trauma, and may lead to osteonecrosis, non:union:, mal:union:, and post-traumatic osteoarthritis. Objective: To evaluate clinical and radiographic outcomes after surgically treated talar fractures and assess whether the timing of reduction and definitive fixation was associated with those outcomes. Methods: This retrospective observational study included 38 of 53 patients who underwent surgical treatment for a talar fracture between April 2013 and April 2017. Demographic, injury, fracture, and treatment data were obtained from medical records. Radiographs and computed tomography scans were reviewed for :union:, mal:union:, osteonecrosis, and osteoarthritis. Functional outcomes were assessed using the American Orthopaedic Foot &amp; Ankle Society (AOFAS) ankle-hindfoot score, visual analogue scale (VAS) for pain, and Manchester-Oxford Foot Questionnaire (MOXFQ). Mean follow-up was 38 months (range, 11&amp;ndash;70 months). Results: The cohort included 33 men and 5 women with a mean age of 33 years. Osteoarthritis occurred in 52.6% of patients and osteonecrosis in 47.3%; mal:union: and non:union: occurred in 5.3% and 2.6%, respectively. Mean AOFAS, MOXFQ, and VAS scores were 60.3, 67.6, and 4.9. Within the observed surgical interval, we were unable to detect a statistically significant association between treatment timing and osteonecrosis or functional outcome. Conclusion: Complications and functional impairment were common after surgically treated talar fractures. The timing finding should be interpreted cautiously because of the limited sample size and potential selection bias.</abstract>
